To help us be as efficient as possible please visit our HIPPA compliant secure server to complete your Health History and Insurance information prior to your first dentist visit.This enables us to provide a 100% paperless dental practice as we do our part to GO GREEN. You DO NOT need to PRINT the E-Forms to bring in with you for your dental appointment. Once your information is submitted, it is sent directly to our server. When you visit our office for your appointment you will be given the opportunity to sign your forms electronically. Thank you for your cooperation.Please be sure to fill out these forms prior to your appointment or else your appointment will be rescheduled.
